NDIS stories: Patrick

A close up of an Aboriginal man's face. He is in his 60s and smiling.

When Patrick found himself unable to leave the hospital in his remote outback town due to a lack of available support, the prospect of living an independent and fulfilling life seemed out of reach. With CASPA’s dedicated NDIS team stepping in, Patrick’s story has transformed into one of hope.

Through CASPA’s support, Patrick was able to transition from hospital care to a supported living environment in the community. For the first time in years, Patrick can enjoy the quality of life he deserves. Recently, his journey took an exciting turn as a family member moved in with him, and together they are planning a family getaway, an incredible milestone for Patrick.

Patrick’s story is an single instance that demonstrates the broader impact of CASPA’s work with the NDIS. For participants in rural and remote locations where support options are scarce, CASPA is working to ensure no one is left behind. This includes establishing new services in remote areas to meet participants’ unique needs, fostering greater independence, and creating opportunities for connection.

In addition to providing vital support for participants with disabilities, CASPA continues to help individuals transitioning from Out-of-Home Care (OOHC) into adulthood. Extending NDIS services to these young people helps them maintain critical social and support networks, paving the way for a smoother journey into independent living.

CASPA’s collaborative and participant-centered approach has been widely recognised by stakeholders. One shared, “The care coordinators at CASPA have gone above and beyond with supporting our team and our mutual Participant—they would make an amazing caseworker!”

The role of CASPA’s team members like Ema has also been invaluable. Her expertise and approachability have been praised, with stakeholders noting her responsiveness to NDIS-related questions and her unwavering commitment to helping young people succeed.

Patrick’s story is an example of how CASPA supports people with disabilities to reach their goals, live independently, and enjoy an improved quality of life. By expanding our services and tailoring our approach to meet the unique needs of each participant, we strive to help individuals achieve their own goals and enjoy the life they want.
